As you can see in the pictures, everything works.  There are 2 flaws I want to point out.  First, you can see some flaking off of the padding material on the headband portion of the helmet.  Second, I think the optical coating has come off of the lenses, and there may be some very hairline scratches, but having played this myself, I can tell you this doesn't seem to impact gameplay.  The stand has a Fallout 4 Vault Boy sticker on it, so if you con't like Vault Boy, you can scrape it off.  Everything comes in one large box, and the helmet is in a box-within-a-box.
